Objective evaluation with a comprehensive, in-home assessment.  Assessment focuses on healthcare status, personal care, safety, legal, financial, psychological and social needs as well as a COVID 19 plan.  Development of a custom, detailed plan of care addressing areas of concern with recommendations. Written report addressing current status and recommendations. 

 

Arrange and implement client specific plan of care.  Care coordination with medical, home health and other service providers.  Facilitate communication between care providers and families. Written reports detailing all care management activities. MAP CARE Management can act as your "Eyes and Ears" and address the challenges that may occur.  

 

Coordinate client appointments, referrals and resources with family and service providers.  Attend medical appointments as needed to facilitate communication.  Maintain consistent communication with emails, calls and written reports.
